Always Read the Fine Print

Mistakes happen.. It's when additional mistakes land on top of the original ones that our catastrophes begin to take shape.   This I know from life experiences.

Like in this situation.

A new hire at the research facility mistakenly put a bottle of experimental cleaning fluid onto a skid that had regular cleaning bottles on it..  That skid got delivered to a distribution center in Buffalo New York where from there it was subdivided per orders to several other hardware suppliers...  The school in question bought them regularly and Fred, the school janitor, used them regularly as a result.  That's how he grabbed what looked like a regular bottle of cleaning concentrate to be mixed with his mop water in order to clean the floors as usual.

But he didn't read the fine print on this bottle that looked exactly like all of the hundreds of other bottles that he had used before.   Now, he is at the mercy of others.

The biggest killer I have found.. Is common everyday routine.. It makes us careless.
